all areas. Don't forget to include a copy of your insurance card. Packets will be available

TRANSPORTATION FORMS
Due to budgetary restraints, there will be no busing for away games. Players/Parents are responsible
for transportation to and from games. If a parent is carpooling other players to and from games
or a player is driving themselves to a game, the following forms must be filled out,
signed and turned in to the front office:

for parents carpooling other players - parents must be cleared by the district
